چکیده

Electroconvulsive therapy (ECT) is a useful yet controversial mode of treatment. Despite being in use for over 70 years, its mechanism of action (MOA) is still not clearly understood. Various theories on MOA of ECT have been proposed over the years. These have included looking at psychological, neurophysiological, neurochemical, neuroendocrine, and neuropeptides mechanisms. An attempt is made here to review the existing literature on the MOA of ECT in a historical perspective. Various theories are discussed in context of the available pre-clinical and clinical research evidence with highlighting of the methodological issues and looking at the relevance of each theory. In summary, there is no single theory that satisfactorily explains the MOA of ECT in various psychiatric illnesses. The need for properly designed, ethically sound pre-clinical and clinical studies is highlighted (German J Psychiatry 2005; 8: 70-84).

The effect of electroconvulsive therapy on the levels of oxidative stress factors in the prefrontal cortex of depressed rats

Background and Objective: Electroconvulsive therapy (ECT) is one of the effective and less complicated methods for treatment of depression in cases of resistance to common treatments. Given the fundamental role of pre-frontal cortex on changing the mood of depression-related behaviors in depressed patients, the effects of electroconvulsive therapy on enzymatic activity of this cortex were taken...

Effects of Lidocaine on the Acute Hemodynamic Response to Electroconvulsive Therapy

ABSTRACT Electroconvulsive therapy (ECT) has been used widely to treat psychiatric disease. ECT under anesthesia is associated with hypertension and tachycardia. The cardiovascular effects of ECT were studied after per-treatment of 100 patients with lidocaine 1.5 mg/Kg (experimental group) and saline solution (control), using a double-blind, randomized clinical trial study. There were no sign...

How does electroconvulsive therapy work? Theories on its mechanism.

This article reviews 3 current theories of electroconvulsive therapy (ECT). One theory points to generalized seizures as essential for the therapeutic efficacy of ECT. Another theory highlights the normalization of neuroendocrine dysfunction in melancholic depression as a result of ECT. A third theory is based on recent findings of increased hippocampal neurogenesis and synaptogenesis in experi...
